Chapters Transcript Video New Healthy Aging Clinic at UC Davis Health mm hmm. The healthy aging clinic here at U. C. Davis was established so that we could develop a clinic that has specific expertise in geriatrics and gerontology we specialize in the care of older adults. This is a multi specialty clinic. So in that way we're unique U. C. Davis health is really committed to becoming an age friendly health system and I feel like the healthy aging clinic is a vital piece of that puzzle. The interdisciplinary nature of care for the aging click is essential. It's built upon a one very important premise but the care of the aging person and their carers requires a number of different approaches. We have an interdisciplinary team of professionals, geriatricians, neurologist, neuropsychologist, neuropsychiatrist nurse practitioners, family caregiving experts and we've brought that expertise together in one clinic so that we can work collaboratively with older adults and their family caregivers to address the complex health problems that sometimes accompany aging. The services I provide are related to cognitive assessments, doing a full neurological assessment. Really looking for um dementia, mild cognitive impairment. One of the nice things that we're able to do in the dementia clinic is take time with people. So my new patient visits I have an hour and if it takes a little bit more than that then it takes more than that a lot of our patients have geriatric syndromes like impaired cognition dementia gait instability and falls. We have daily huddles where we can talk about the patients already. We're going to start. I know that my fellow physicians and providers are down the hall. So if I have a quick question I can ask them. We talk to each other about complex patients people where we may have a sense that there's extra going on that's outside of our specialty. Having everyone under the same roof can really streamline it. Not only do we do comprehensive geriatrics assessments we also have the Family Caregiving institute which is really beneficial and unique to UC. Davis. When dr Lubarsky said that he wanted U. C. Davis health to create a world class geriatrics program that included family caregiving. It was music to my ears here um in the healthy aging clinic I see family caregivers one day a week. And so by taking care of caregivers by partnering with family caregivers I think we're able to deliver a higher level of care to the older adults that we care for. This is a great opportunity to really show how this health system can excel in bringing aspects of care together under one roof and really improve the quality and access to care of a very important population. We want to provide comprehensive care to community dwelling older adults. We want to maintain their functionality, their independence so that they can live their healthiest lives Created by
